CONTINUUM OF CARE (CoC)
HUD Continuum of Care Homeless Assistance Funding Available
The NY-518 CoC Board invites proposals from not-for-profit or government entities in Oneida and Madison counties for the following grants: Collaborative Applicant (CA), Coordinated Entry (CE), and/or Homeless Management Information System (HMIS) grants. Each grant is for a three (3) year term. Funding for the grants is provided through the annual NOFO published by the U.S. Department of Housing and Urban Development (HUD). Any award is contingent upon the availability of funding from the U.S. Department of Housing and Urban Development. Funding for these grants is determined by the annual NOFO.
In 2025, HUD awarded $219,297 for the Collaborative Applicant Planning Grant, and $255,766 for the Coordinated Entry Grant (with a 25% match on the CE grant), and $329,868 for the Homeless Management Information System (HMIS) Grant with $67,000 from program fees for a total of $329,868.
An eligible entity may apply for one, two, or all three of the grants.

General Instructions
The full package must include a narrative response, a budget, and a narrative explanation of that budget. Nonprofits must submit their IRS letter of determination and a copy of their most recent audit, including a management letter if one was issued. Documents will be submitted electronically. Please combine the narrative response and budget documents into a single PDF. IRS letter and audit documents could be included as separate documents. See attached RFP application for details regarding that grant.
Subject line should read “CoC Collaborative Applicant RFP Submission”.
Applicants will receive confirmation that their submission was received and is under review.
Proposals emailed after the deadline will not be accepted. Proposals may be withdrawn or modified following the same manner of submission indicated herein until the application deadline.

The Mohawk Valley Housing and Homeless Coalition, CoC NY-518, is a membership-based coalition whose mission is to prevent and end homelessness in Oneida and Madison counties. We achieve this through fostering shared responsibility, collaborative planning, and aligning the stakeholders and resources essential to implementing local strategic priorities to prevent and end homelessness. The CoC NY-518 coordinates the implementation of a housing and service system that meets the needs of all persons experiencing homelessness throughout its geography. The homeless response system includes: • Outreach, engagement, and assessment; • Homeless prevention and diversion strategies; and • Emergency shelter, permanent housing, and supportive services. Ultimately, the CoC NY-518's vision is to make: • Homelessness rare in the Utica/Rome/Oneida & Madison Counties, and • The experience of homelessness brief, and a one-time occurrence.
